This Issue Is Resolved With Dragon Version 12]
The latest Adobe Flash update (11.3.300.257) may result in the infamous “COM returned an unexpected error code: Details are HOOKERR_NONOTIFYWINDOW” Dragon error message for some individuals. If so, then of course, Dragon will be unusable in most, if not all, applications, once the error is triggered.
If you are unfortunate to find yourself in this situation (around June 2012), the above error results after loading the web browser, Mozilla Firefox, where Flash has been used. Unfortunately, most websites have some Flash on them, so that will be difficult to avoid.
Running Flash directly will not result in this error, indicating there is likely an issue between recent versions of Flash and Mozilla Firefox that affects Dragon NaturallySpeaking NaturallySpeaking version 11.x, Dragon Medical 10.x.
Until there is a better fix, we suggest one of the following:
- Uninstall Adobe Flash 11.3.300.257 and install Adobe Flash 10.3. Adobe Flash 11.2 has currently exploitable security risks (thank you, Bernd for this additional information).
- Disable flash in your web browser: Firefox menu> Add-ons> Plugins — then select Shockwave Flash and Disable.
- Try using Internet Explorer or Chrome (please report to us if you see this error message when using Internet Explorer or Chrome).

We will append this post with more information as we have it.

We cannot duplicate the error message with Dragon Medical Practice Edition. If you are using DMPE and receive this error, please comment here.
This issue seems to be isolated to non-medical versions of Dragon 11.x.

Nuance has indicated that a specific OS may also be a factor. I’m hoping to learn more as they do, and as always, welcome further input from others.

Bernd von Kraut (thank you, Bernd) has provided some additional information by way of a comment below which I copy and paste here:
Version 11.2 of Flash has some severe security flaws. I recommend to install version 10.3  which is still maintained by Adobe for enterprise customers .
